System Navigator

Description
The Housing Stability area of Hennepin County's Health and Human Services is seeking multiple Systems Navigators to be part of a 5-person housing-focused systems navigation team. These positions will work through the complexity of unsheltered homelessness by conducting assessments of encampments and individuals occupying encampments and brokering necessary services through a trauma-informed, person-centered approach to ensure that households are deeply supported to resolve their housing crises as quickly as possible and to find safe, appropriate alternatives to sleeping outdoors in an encampment.

Location and hours:
These positions are hybrid and will be performed both on-site at the Hennepin County Government Center at 300 South 6th Street, Minneapolis, MN 55487, community sites within Hennepin County, and remote as job duties require. While these positions are designated as hybrid, based on current requirements hires must reside in Minnesota or Wisconsin. Work hours are Monday through Friday, 8 a.m.- 4:30 p.m. including evenings and weekends.

New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

About these position types:
Theses are limited duration (temporary) full-time, benefit earning positions expected to last up to two years with the possibility of extension.

In these positions, you will:
  • Engage with people experiencing homelessness in encampments in a person-centered, trauma informed way on a regular basis to conduct housing and other needs assessments and to identify and navigate the homelessness response system to identify a safe, appropriate alternative to sleeping outdoors including shelter and housing.
  • Broker services and support unsheltered households to access and connect to referrals, including providing transportation, emotional support, and necessarily follow up.
  • Work closely with Healthcare for the Homeless, the Hennepin County Homeless to Housing teams, and community street outreach partners to coordinate services for identified households.
  • Work with the homelessness response system(s), and ancillary systems including adult behavioral health and family, and youth systems, to be conversant in all the housing solutions that may exist or be available to reduce barriers and resolve housing crisis.
  • Meet with neighborhood and mutual aid groups to ensure coordination of mutual aid and volunteer activities within encampments.
  • Track and record data in Homeless Management Information System (HMIS) and other program information tracking systems using established workflows.
Need to have:
  • One of the following:
  • Two years or more of responsible, paid social service experience providing direct client service
  • A two-year human services certificate
  • Note: College coursework in the behavioral sciences can be substituted for the required work experience
  • Access to reliable transportation for travel to various shelters and drop-in centers throughout Hennepin County. 
  • Ability to work a flexible schedule including evenings and weekends.

Nice to have:
  • A valid driver's license.
  • Experience:
  • Working directly with individuals facing homelessness.
  • Working with Homeless Management Information System (HMIS).
  • Using Microsoft Suite.
  • With databases and/or online tools.
  • Familiarity of Trauma Informed Care, Motivational Interviewing, and Assertive Engagement.
  • Work effectively with external and internal partners. 
  • Communicate effectively both written and verbally.
