A unique opportunity!

The Southwestern Wisconsin Community Action Program (SWCAP) is looking for a Project Manager to plan and implement the Pathways Community HUB model of community health in southwestern Wisconsin. Please see https://pchi-hub.com/ for a complete description of the Pathways Community HUB model.

SWCAP is working to establish a network of community partnership involving government, healthcare, healthcare insurance and nonprofit organizations. Our intent is a network that acts to improve the connection between people in southwestern Wisconsin that have difficulty accessing healthcare resources, and the providers of those resources. The goal is to improve overall community health and wellbeing, especially among SWCAP’s core clientele of people experiencing economic hardship. SWCAP is intending to implement the nationally recognized Pathways Community HUB model of community health to establish and maintain this community partnership network.

The Pathways Community HUB Project Manager is the point person for developing and carrying-out a work plan to establish this network in southwestern Wisconsin. We see this Project Manager as someone with an entrepreneurial attitude that has knowledge of healthcare, healthcare funding, human services or public healthcare, and is passionate about health and wellbeing for people of all socioeconomic backgrounds. With a successful implementation, the Project Manager could transition into a position of managing southwestern Wisconsin’s Pathways Community HUB on a regular basis.

SWCAP is a 501(c)(3) nonprofit organization that operates in the Wisconsin counties of Grant, Green, Iowa, Lafayette and Richland. The Pathways Community HUB Project Manager will be based in SWCAP’s Neighborhood Health program that is located in Platteville, Wisconsin.
