Nursing - RN or LPN

The function of the nurse is to promote optimum development and health of the people we serve and assist other staff in meeting regulations & licensing.
     
ESSENTIAL FUNCTIONS:    
  • Monitor the health status of the individuals who have been assigned to your care.
  • Report changing health conditions to appropriate team members.
  • Develop a health care plan, and incorporate it into the CSSP (Coordinated services & support plan)
  • Participate in the periodic review of the CSSP as a member of the interdisciplinary team.
  • Teach and counsel the individuals we serve and staff in the areas of health and disease and promoting self-responsibility and independence whenever possible.
  • Arrange for appropriate medical and remedial services and incorporate recommendations into the CSSP.
  • Implement all health service policies and procedures in compliance with the state and/or federal regulations.
  • Advocate for an awareness of the needs and changing life-styles on the developmentally disabled persons in the community.
  • Carry and respond to calls on the nurse emergency cell phone on a rotating basis.
  • Assist in instructing the staff in medically related orientation classes and annual continuing education needs.
  • Assist with the development of individual/household health or medical related protocols and procedures.
  • Follow agency guidelines and regulations.         
  • Attend and participate in regular staff meetings/in services deemed mandatory.
  • Communicate effectively and work cooperatively with co-workers and administrative staff members.
  • Occasionally be assigned or asked to assist with co-workers’ homes. This would occur when staff is unable to reach the nurse assigned to their individual.
  • Complete work consistently and in a timely manner.
  • Process paperwork promptly, accurately, and thoroughly with attention to detail.
  • Complete other duties as assigned.  

JOB SPECIFICATIONS:    
  • Must have a current license in the state of Minnesota
  • Sensitivity to individual differences.
  • Valid driver's license and driving record that meets the requirements of Hammer Residences insurance carrier.
  • Excellent written and verbal communication skills including fluency in English and proficiency with Microsoft Office (especially Outlook and Excel) and SharePoint
  • Requires strong organizational skills; frequently organizes multiple projects at once, requiring excellent organizational skills, time management, close attention to detail, and proven ability to achieve assigned deadlines
  • Demonstrate a strong commitment to confidentiality and customer service.
  • Requires ability to work with people of different abilities, ages, cultures and ethnicities
  • Adhere to a strong ethical code in compliance with organizational mission and values
Physical demands include frequent use of a telephone, frequent use of repetitive motion activities including typing on a computer keyboard and monitor, laptop or scanner and the ability to work for several hours at a time at a desk and computer. This job description assigns essential functions. Schedule: M-F Schedule. Nurses share the responsibility for taking the emergency call rotation and being on-call for one week every 6 weeks.

Wage: Depending on years of experience and education.
