Case Manager – Collaboration of Housing Resources (COHR)

POSITION SUMMARY: The Housing Case Manager with the Collaboration of Housing Resources (COHR) Team serves single adults experiencing long-term homelessness, in scattered site apartments in the community. This position will be responsible for housing search and landlord relations, as well as rule 79 Adult Mental Health Targeted Case Management and subsidy management. This team utilizes Housing First and Harm Reduction models to assist individuals who have experienced long-term homelessness to find and maintain housing. Bilingual Spanish Speakers encouraged to apply!

ESSENTIAL RESPONSIBILITIES:
  • Conduct an initial assessment and develop an Individual Community Support Plan that will assist clients to address their needs, deficits, and goals. 
  • Visit clients in their home as well as the community on a regular basis to check progress, provide support and monitor mental health functioning and medication effectiveness.
  • Refer clients to appropriate community resources and arrange transportation to appointments as needed.
  • Cultivate relationships with landlords willing to provide housing for participants and assist with administration of rental assistance.
  • Communicate with case management team members, community professionals, and other service providers to ensure that services for client are coordinated.
  • Maintain client files and program records, court reports, and complete reports as required.
  • Participate in staff meetings, clinical supervision, and meet regularly with program coordinator for individual supervision to meet supervision requirements.
  • Comply with HIPPA and Data Privacy Practices.
  • Positively represent agency, program, and participant needs.
  • Actively participate in the agency’s undoing racism plan.
  • Assist in direct supervision of clients during open social / recreation activities at the CSP.
·      This position will require the ability to work outside regular business hours on an occasional basis to meet client and program needs. 

QUALIFICATIONS:
  • Bachelor’s Degree in behavioral sciences or related fields, including but not limited to social work or psychology from an accredited college or university; or Associate Degree with four years experience as case manager associate. Master’s degree preferred.
  • Have at least 2,000 hours of supervised experience in the delivery of services to persons with serious mental illness.
  • Working knowledge of Harm Reduction and Housing First.
  • Experience working with individuals experiencing homelessness.
  • Past experience working with a caseload is preferred.
  • Ability to relate to and advocate for diverse client needs and interests.
  • Ability to complete paperwork accurately and in a timely manner.
  • Ability to learn and understand client needs and mental health recovery related to the provision of community mental health services.
  • Strong written and verbal communication skills and computer experience.

SALARY:  $21.88 per hour DOQ
