Nursing Supervisor, Community Based

THE POSITION
Hennepin County Public Health Department is seeking a Community Based Nursing Supervisor to manage, develop, implement, and evaluate Tuberculosis (TB) Case Management Services. This leadership position will lead a team providing case management services including directly observed therapy to patients with TB infection. The TB case management team is an interdisciplinary team of professionals which includes both Community Health Workers and Public Health Nurses. This position represents the department to other professionals in similar programs in the community, other county & state jurisdictions, and to the general public.

Location and hours:
This position is located at the Hennepin County Public at 525 Portland Avenue, Minneapolis, MN 55415. Normal clinic hours are Monday and Wednesday 8 a.m. to 7 p.m., Tuesday and Friday 8 a.m. to 4 p.m., and Thursday 10 a.m. to 4 p.m. Hours for this position are typically Monday through Friday 8 a.m. to 4:30 p.m.

This is a full-time, benefit earning position.

In this position, you will:  
  • Establish and maintain standards for quality patient care and work through coaching, training, and evaluating staff performance.
  • Work with staff to provide information, referral, outreach, assessment, crisis intervention, and service coordination.
  • Facilitate continuous process improvement of systems and programs by determining needs and improvements; develop plans, policies, and procedures; and evaluate outcomes for programs and services.
  • Work with a diverse team to maintain a respectful work environment to develop team goals, assess workforce needs, establish performance outcomes, maximize performance results, plan for implementation of programs, and prepare schedules for completion of work.
  • Serve as program lead and TB subject matter expert to ensure program compliance with applicable state and federal guidelines.
  • Coordinate with Hennepin County Accounting, manage program grant budget, monitor expenditures and revenue; prepare required reports and invoices for various funders including state and federal entities.
  • Participate as an active member of the Public Health Clinic management team and the larger Public Health managers/supervisors team; serve as representative of the department, service area, and Public Health Clinic to internal and external audiences; may serve in key role for Public Health Emergency Preparedness Incident Command.
 
Need to have:
  • Bachelor's degree or higher from an approved school of nursing and two or more years of professional nursing experience.
  • Licensure as a Registered Nurse issued by the state of Minnesota.
  • Ability to work at various county locations on a regular basis and access to reliable transportation.

  • Experience:
  • In a supervisory role.
  • Providing leadership to a team or peers; supervisory experience preferred; ability to provide work direction to professional and paraprofessional staff.
  • Developing plans, goals, resources, and managing public health strategies and programs.
  • Working with patients diagnosed with TB.
  • With the development, writing, and management of public health grants.
  • Working within multidisciplinary workgroups and management teams.
  • Knowledge of:
  • Public health, Nursing, TB infection, treatment protocols, and case management programs.
  • County and community resources, practices of public health nursing, and public health ethics.
  • Ability to:
  • Manage budgets, set goals and objectives with timelines to achieve results, exercise independent judgment and initiative to accomplish work.
  • Prepare and submit accurate and concise reports; effectively communicate in verbal and written form.
  • Foster an environment that recognizes and respects cultural, lifestyle, and socioeconomic differences.
