Job Coach


POSITION SUMMARY: The Job Coach will assist individuals with disabilities and other barriers in retaining competitive employment and maintaining mental and physical health stability, through long term follow up supports for participants.

ESSENTIAL RESPONSIBILITIES:

  • Provide direct monthly contacts with program participants (both in person and via phone), and follow up contacts with their employer, as needed.
  • Counsel program participants on job issues and develop creative retention strategies.
  • Teach job tasks and coach participants on behavioral/interpersonal skills.
  • Resolve and mediate employer/employee conflicts utilizing problem solving skills and ADAAA knowledge.
  • Complete documentation of contacts and activities with program participants.
  • Collect documentation of hours and wages of program participants and submit as program specifies.
  • Provide weekly case notes and/or monthly reports, as needed.
  • Market job retention services to increase referrals into program.
  • Complete weekly and monthly productivity summary reports for submission to the program manager.
  • Manages miscellaneous projects and completes various tasks as needed by management.
  • Work cooperatively with other team members to ensure a smooth transition of services
  • Attend all staff meetings.
  • Strive to implement all Avivo mission and policy standards.

 
QUALIFICATIONS:

  • Post secondary education in human services or related field. Degree preferred.
  • Customer Service experience.
  • A desire to help individuals with disabilities and other barriers maintain employment.
  • Ability to work within ADAAA guidelines.
  • Excellent time management and organizational skills.
  • Excellent written and verbal communication skills.
  • Ability to work independently.
  • Ability to work a flexible schedule to meet the needs of consumers.
  • Travel independently throughout the Twin Cities area.
  • Hear and speak in order to communicate with program participants.
  • Work is performed in varied environments including office, warehouses, indoors and outdoors in the community.


WAGES:  $18.27 per hour
