Per Diem Speech Language Pathologist
We are a reputable locally-owned company specializing in home-health. We treat our patients with compassion and respect by taking time to get to know the people we care for. We are looking for a per diem Speech Language Pathologist to work within Pine Crest Nursing Home in Merrill, WI.
Summary Statement: The Speech and Language Pathologist evaluates and treats patients in accordance with orders from a physician through use of therapeutic procedures designed to address deficits in speech, language, hearing, swallowing and cognition and improve independence, safety and quality of life. The Speech and Language Pathologist participates as a member of the interdisciplinary team providing support and information within the area of speech therapy.
- Conducts and documents a thorough functional assessment of the patient utilizing standardized procedures, including home/vocational and equipment needs
- Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals
- Record patient progress in the patient record a minimum of every ten (10) days
- Documents the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format
- Orient patients to program services and treatment procedures appropriate to their needs. Inform patients of any potential risk during any procedure
- Assist in the referral process when a patient requires additional services following discharge.
- Establish and instruct patient and family/other care giver in exercises and therapeutic procedures to be continued following discharge
Education Requirements:
- Graduate of an approved program in Speech/Language Pathology curriculum
- Current license/certification in Wisconsin
- American Heart Association CPR Certified
